Website Privacy Policy & HIPAA Overview
Effective date: January 1, 2025 · Practice: Millmark Counseling
Strict separation of website traffic and Protected Health Information (PHI)
This Privacy Policy describes the practices of Millmark Counseling regarding information collected through our public marketing website (). Please note: once an individual becomes an active patient, health records and clinical information are governed exclusively by federal HIPAA privacy laws and our formal Notice of Privacy Practices (NPP), provided through our certified Electronic Health Record (EHR) portal.
1. Information collected on the public website
We deliberately minimize data collection on this website to safeguard your privacy:
- Voluntary contact information: basic contact coordinates such as your name, email, phone number, and non-sensitive scheduling preferences.
- Technical log data: standard server logs (truncated IP addresses, browser user agent, timestamp) strictly for technical security and denial-of-service prevention.
- No third-party advertising trackers: we do NOT employ advertising tracking pixels such as Meta Pixel, Google Ads remarketing, or TikTok trackers.
2. Prohibition of sensitive PHI in public forms
Our public contact forms are designed strictly for initial scheduling logistics. We instruct visitors not to enter sensitive psychiatric diagnoses, detailed symptom histories, medication lists, or social security numbers. Sensitive clinical data must be communicated only via encrypted EHR intake forms or during private clinical sessions.
3. How we use submitted information
Information submitted through our inquiry forms is used solely to:
- Respond to your initial questions and schedule a consultation call.
- Send secure intake invitations if mutual consent to proceed is established.
- Comply with applicable statutory or legal mandates.
We never sell, rent, trade, or distribute your personal contact information to commercial data brokers or marketing affiliates.
4. HIPAA Notice of Privacy Practices summary
Under the Health Insurance Portability and Accountability Act of 1996 (HIPAA), active patients have specific rights regarding their Protected Health Information (PHI), including the right to inspect their medical records, request amendments, receive an accounting of disclosures, and request confidential communication channels. Complete, signed copies of our Notice of Privacy Practices are furnished to all clients prior to their first intake session.
